Maybe the eligibility rules change. Maybe the match formula changes. Maybe the plan switches to a different safe harbor formula. Maybe compensation is redefined. On paper, everything is perfect.

Then payroll keeps doing exactly what it was doing before.

That is the problem.

A retirement plan document is not just paperwork that sits in a file until the next restatement. It contains the rules the plan is supposed to follow. If those rules change, the people handling payroll, enrollment, contributions, testing, and administration need to know about it.

Too often, the amendment gets handled at one level while operations remain in the dark.

Maybe HR never gets a copy. Maybe payroll is never reprogrammed. Maybe the recordkeeper was told, but the TPA was not. Maybe everyone assumes somebody else handled it.

Then a year or two later, somebody discovers that the plan has been operating under the old provision the entire time.

Now you have a correction problem.

The frustrating part is that these errors are usually preventable. Every amendment should trigger an operational checklist. What changed? When is it effective? Who needs to know? Does payroll need to be updated? Does enrollment need to change? Does the recordkeeper need new instructions?

Signing the amendment is only half the job.

Plan sponsors need to understand that document changes have to make it from the conference room to the payroll system.

A beautifully drafted amendment does absolutely nothing if nobody actually administers the plan according to it.

The document changed.

Operations never got the memo.

And that is how a paperwork exercise becomes an operational failure.

One of the easiest ways to create a 401(k) operational failure is to amend the plan document and then forget to tell the people actually operating the plan.
